DTR — 0 absent · 1+ hypoactive · 2+ normal · 3+ brisk · 4+ clonic.

Tone — one arrow for mild, two for marked. ↓ hypotonia, ↑ hypertonia (spasticity or rigidity — specify in Remarks).
